How to apply

Transport to Treatment is available to support you while you are navigating cancer if you are facing hardship or finding it hard to get to your treatment.

1. Check your eligibility for the service

Chat to the healthcare team at your hospital or treatment centre about your eligibility for the service and the designated services areas.

The service user must:

  • Have a cancer diagnosis.
  • Be receiving active treatment, palliative care or attending a medical appointment related to their diagnosis.
  • Living within a designated service area.
  • Be facing hardship in accessing treatment.
  • Be physically independent, able to mobilise and get in and out of a vehicle safely unassisted. Where a service user utilises a mobility aid, this must be collapsible.
  • Be well enough to travel and does not require medical supervision.
  • Require travel within service hours, depending on travel distances and times.
  • Agree to adhere to the Home to Treatment Guidelines above.
2. Your health professional must lodge a referral form

As a health professional, you can refer someone to this service by:

  • Review the Home to Treatment Guidelines and confirm the service user’s eligibility
  • Determining the service is suitable and will meet the service user’s needs
  • Obtaining the service user’s consent before referring to the service
  • Completing and submitting the online referral form below:

This form will take approximately 5-7 minutes to complete. There are 37 questions in total, and it’s easiest to complete the form when you are with your client.

3. Application assessment

Once your referral has been received, you will be contacted by Cancer Council Queensland Information and Support service for an assessment and to discuss your transport needs.

4. Transport set-up

The Transport to Treatment service uses vehicles with Cancer Council Queensland branding. Our vehicles are operated by our generous volunteer drivers and rely upon their availability. The service isn’t guaranteed, so we recommended a back-up option if the service is disrupted or delayed. A carer or support person may accompany you in the vehicle to your treatment appointment.
